The off-site choice also requires an address on "address line 1" in Sunflower.

• If the location is a business, please include that address information.
• If the location is a private residence, please use "on file" and have the address information on the off-campus form filed with the DPA. This information can be viewed in public reports, so for privacy concerns we do not want personal residences listed in the on-line system.
