If your sponsored project activities were impeded by COVID-19 precautionary measures and/or illnesses, complete a COVID-19 Sponsor Notification and Request Form. Use the step-by-step guide to access and attach the form to a SeRA Central Office Request - COVID-19 Sponsor Notification/Request. ORA, RMG, or ICO Institutional Officials will work with you and notify the sponsor, and request needed approvals. The form is for federal and non-federal sponsored projects.
